1 Some source contradict each other. While it is known that people who bear the surname Qin are descendants of Qin royalties, there are also sources mentioning Qin nobles with surname Zhao. The case of Zheng is a famous example. EDIT : While some source mention Feizi (first Qin ruler) with the surname Qin, he was also part of Zhao clan.
2 (x)is simplified version of used in the mainland. I prefer to use traditional version used in Taiwan to avoid confusion since most chinese wikipedia page use traditional version.
3 Different source give different name. 姒 Si is taken from 史記 Shiji, 芈Mi is from 國語 Guoyu.
4 Some surname have multiple origin. After unification, the meaning of 姓 and 氏 blurred. The modern meaning of 姓 is “surname” or “family name” and 氏 slowly replace 姓. Of the at least 6 姓, only two remain widely used as “surname” 姜 Jiang and 姚 Yao.

more might be added later, there are a lot of them (some source indicate that there are hundreds but most of them are unknown).

Si 姒 is the surname of Xia dynasty rulers.
Zi 子 is the surname of Shang dynasty rulers.
Ying 嬴 is the surname of a famous Shang general Fei Lian 飞廉. Qin rulers are his descendant through his son E Lai 恶来(who died early) and Zhao ruler are his descendant from his other son Ji Sheng 季胜.
姬 Ji is the surname of 周 Zhou rulers. Notice that majority of the states bear the ancestral name 姬 Ji. This is because it’s founder are brothers of 周 Zhou kings (of various period).

The diagram below is taken from a Chinese website, hence in simplified Chinese. I would not add the pinyin, it’s too much work.
